What happens when my provider leaves Circle Medical?

Learn how we seamlessly transition your care to a new primary care provider (PCP) without interrupting your treatment, records, or prescriptions.

Providers sometimes leave for personal or professional reasons, but this doesn't affect the quality of your care or your access to your health information. Here's what you need to know if your PCP has unexpectedly changed.

Meeting your new primary care provider (PCP)

If your primary care provider (PCP) is no longer with Circle Medical, we'll assign you a new provider so your care continues without a gap. You’ll see your new provider's name in your account, and will be notified of this update via email. If you prefer, you can change your PCP to a provider of your choice.

How do I choose my own PCP?

  • To choose your assigned PCP, use our provider search tool to view providers based on availability, specialty, and insurance, then book your appointment with whoever fits best. Then, during your appointment with your preferred provider, ask them to update your assigned PCP in your account.

  • Or, start a chat on the mobile app or web portal, and the Care Team can change your assigned PCP to a provider of your choice.

What about my medical history, prescriptions, and future appointments?

Your medical history stays with us

All your records, including past appointments, notes, and test results, stay in your Circle Medical account. Your new provider can review your full history before your next appointment, so you won't need to repeat your story or redo any forms.

Your prescriptions and refills continue

Your new PCP will review your current medications and can continue your prescriptions, including refills. If you take a controlled substance like a medication for ADHD, your provider may request an additional appointment to discuss your treatment plan.

Your new provider may ask for a fresh review

Every provider is responsible for the care they give you. Your new provider may ask you to repeat an assessment or book an extra appointment so they can review your history with you. This is their clinical judgment, and it's there to make sure your care is right for you.
