Below, you can find department contact information and instructions for sending records from an outside healthcare provider or submitting them yourself.

Submitting Your Own Medical Records
If you have electronic copies of your medical records, you can upload them through the mobile app or the website portal by following the instructions below:
Open the Circle Medical app OR our web portal.
Tap the “Health Records” tab (located along the bottom navigation bar for mobile) OR Click the "Health" tab (found at the top of the page for web).
Select the “Prior Medical Records” option.
Tap the blue “Add / Request Record” button.
Select “Upload”.

If you do not have electronic copies of your medical records, you can contact your external healthcare provider or medical facility directly to obtain them. Once you have the records, you can follow the instructions above to send them to Circle Medical.
If you are experiencing any technical issues and need support, you can always reach out to one of our Patient Care agents. In some cases, you may be directed to share your records via email. Make sure to include your name and any other relevant information that we may need to identify you in the email so that your records can be matched to your chart quickly.
Sending Records from an Outside Healthcare Provider
Reach out directly to your outside provider to request for your medical records to be sent to Circle Medical. Alternatively, you can submit a request through our app for Circle Medial to retrieve your records for you. Here's how:
Open the Circle Medical app or our web portal.
Tap the “Health Records” tab (located along the bottom navigation bar for mobile) OR Click the "Health" tab (found at the top of the page for web).
Select the “Prior Medical Records” option.
Tap the blue “Add / Request Record” button.
Select “Request from Prior Facility”.

If you opt to coordinate the transmission of your records with your outside provider directly and they are sending the records via fax, make sure they use Circle Medical's fax number: (888) 803-3331.
If your external healthcare provider is sending the records via email, make sure they use Circle Medical's email for medical records: [email protected].
