How to get your own medical records

How to download one appointment note or request a copy of your chart, how long each step takes, and what to do if the download link expires.

You can download a single appointment note yourself, or ask the support team for a copy of your Circle Medical chart. The chart can be one note, a result such as a sleep study, or your records as a whole.

This article explains each way to get a copy, how long it usually takes, when to reach out, and what to do if the download link expires.

How long does a copy take?

Each request has its own clock.

  • One appointment note: your provider usually finalizes it within 24 hours of the visit. After that, you can download it from the appointment.

  • A copy from the support team: usually processed and sent within 3 business days. A large chart can take longer. If we need more detail, the support team will message you in chat.

  • The download link: expires 7 days after it is sent. If it expires, start a chat and ask for a new link.

Download the appointment note yourself

  1. Log in to the Circle Medical app or web portal.

  2. Open the Appointments tab.

  3. Select the date of the appointment.

  4. Download that appointment's notes as a PDF.

This downloads that appointment note only. It does not download your full chart.

If the appointment note is not there yet

Appointment notes are usually finalized within 24 hours. If a note is still missing after 48 hours, reach out to support.

Request a copy of your chart

  1. Log in to the Circle Medical app or web portal.

  2. Start a chat with the support team.

  3. Select Medical Records and follow the prompts to select the records you're looking for. Let support know if there is any information you need redacted as part of the download.

  4. Say how you want them delivered: a link in the HIPAA-compliant chat, or a password-protected link by email.

  5. Verify your identity when the support team asks.

  6. Open the secure link and download the PDF.

Important note: The link expires in 7 days. Do not share it or forward it. It is for you.

If the copy is emailed, the file is password protected. The email explains how to open it.

If you need a letter, not the chart

A download of your chart is not a letter your provider writes. Diagnosis letters, treatment summaries, employment letters, and forms such as FMLA are a different request. Your provider decides whether to complete them.

When to contact the support team

  • An appointment note is missing more than 48 hours after the appointment: start a chat and include the visit date.

  • The download link expired or you cannot open it: start a chat and ask for a new link. Do not ask someone else to use the old one.

  • It has been more than 3 business days since you requested a chart copy and you have no link: start a chat.

  • You need the records sent to another provider, not only to you: use How to release your medical records to another provider. Do not forward your download link.

What to do if something goes wrong

The download link expired

Start a chat and ask for a new link. The first link stops working 7 days after it was sent.

I never received a link

Check the chat thread and the email on your account. If it has been more than 3 business days, start a chat. Include the date you asked and whether you wanted the link in chat or by email.

I cannot open the email file

Emailed copies are password protected. Use the instructions in that email. If the password instructions are missing, start a chat. Do not forward the file to get help opening it.

Someone else needs my records

Do not send them your link. An outside provider or organization can email [email protected] or fax a request to (888) 803-3331. Hours for that team are in How to contact Circle Medical.

If you want Circle Medical to send the records, use How to release your medical records to another provider.
