How to share your vitals

When your provider asks for blood pressure, pulse (heart rate), height, or weight, how to enter them on the mobile app or web portal, and how to send a photo instead.

You don't need to send vitals after every appointment. Your provider will tell you during a visit, or ask in your Secure Care Inbox, when they need a reading. This article covers how to enter blood pressure, pulse (heart rate), height, and weight on the mobile app or web portal, and how to send a photo or screenshot if you already have one.

Disclaimer: You must be an established Circle Medical patient and have had an appointment before you can add or upload your vitals.

Do I need to send my vitals after each appointment?

No. Send a reading when your provider asks. They'll let you know the reading they need during your appointment, or message you in your Secure Care Inbox.

If stimulant medications, like Adderall or Vyvanse, are part of your treatment plan, your provider will require blood pressure, pulse (heart rate), and weight at every appointment. Those readings help them confirm the medication is safe for you.

Before you start

  • Have your numbers ready. A home blood pressure cuff, a pharmacy cuff, or a scale all work. You type the numbers in. This form does not take a photo.

  • Add one reading at a time. Blood pressure, pulse (heart rate), weight, and height each have their own form.

  • Pulse (heart rate). These words mean the same reading. In the app, this reading is labeled Pulse.

Add your vitals on the mobile app or web portal

The steps are the same on the mobile app and the web portal.

  1. Log in to the Circle Medical app or web portal.

  2. Open the Health tab.

  3. Select Vitals.

  4. Select Add vital.

  5. Enter the numbers for that reading, then select Done.

You can add your vitals directly from this link as well.

What to enter

  • Blood pressure. Select Add blood pressure. Enter Systolic (the top number) and Diastolic (the bottom number), in mmHg.

  • Pulse (heart rate). Select Add pulse and enter it in beats per minute. A blood pressure cuff usually shows it next to your blood pressure.

  • Weight. Select Add weight and enter it in lbs.

  • Height. Select Add height and enter feet and inches.

Enter the readings your provider asked for.

Important note: If a reading is far outside the usual range, the app asks you to confirm it or check it again before it saves.

What if my reading is out of range?

A number outside the usual range is not your provider's interpretation of it. Our team reviews out-of-range vitals.

If you are concerned, or you have symptoms, seek urgent attention. Call 911 or go to the nearest emergency room. Do not wait for a message from us.

If you are not concerned and you have no symptoms, you do not need to start a chat only because a number is out of range. Our team is already reviewing it and will reach out via your Secure Care Inbox.

Send a photo or screenshot instead

If you already have a picture of your reading, such as a cuff screen, a pharmacy printout, or a visit summary, upload it as a medical record. For the full records guide, see How to share medical records with your Circle Medical provider.

On the mobile app or web portal:

  1. Open the Health tab.

  2. Select Prior medical records.

  3. Select Add / request record.

  4. Select Upload.

  5. On Select record, choose Vitals.

  6. Select Add files, attach your photo or screenshot, then select Upload.

The upload screen says your provider will be alerted. Photos and screenshots are usually reviewed within a few days.
