Sharing medical records is simple on our mobile app and web portal. You can easily upload your records or request that Circle Medical retrieve your records on your behalf. If you have any questions, our Care Team is committed to supporting you along the way.
Sharing medical records with Circle Medical
Coordinating with your external provider to share records via fax or email
If you'd like your external provider to share your medical records with your Circle provider, they can do so via fax or email:
If your external provider is sending records via fax: (888) 803-3331.
If your external provider is sending records via email, ensure they send your medical records to: [email protected].
Request for Circle Medical to retrieve your records from your external provider
Alternatively, you can submit a request on the mobile app or web portal for Circle Medical to retrieve your records for you. Here's how:
Open the Circle Medical app or our web portal
Locate and select the “Health Records” tab
Select “Prior medical records”
Select “Add/request record”
Select “Request from prior facility”
How to upload your medical records via mobile app or web portal
If you have electronic copies of your medical records, you can upload them through the mobile app or the website portal by following the instructions below:
Open the Circle Medical app or web portal
Locate and select the “Health Records” tab
Select “Prior medical records”
Select “Add/request record”
Select “Upload”
Tip: If you don't have electronic copies of your medical records, Circle Medical can request them for you, or you can contact your external provider or medical facility to obtain them.

Step 2: Select the “Prior medical records” option, then select "Add/request record"
