An authorized user is a person you choose to help with your Circle Medical information, such as a family member, caregiver, or friend. They can see only what you mark on the Patient Information Access Authorization Form. You can remove their access whenever you want.
This is separate from sending your records to another clinic. For that, see Medical records release forms.
What an authorized user can access
You decide on the form. You can allow any of these:
Appointment scheduling and reminders
Billing records
Medical records
If you leave an item unmarked, that person does not get that access.
How to add an authorized user
Start a chat with the support team in the mobile app or web portal. See How to start a new chat with the support team.
The support team emails the Patient Information Access Authorization Form to the email on your Circle Medical account.
Complete the form. Include the person's full name and their relationship to you, and mark what they can access.
The support team adds them after you return the completed form.
How to remove an authorized user
Chat the support team from the mobile app or web portal and say you want to remove that person's access. You can also tell your provider during an appointment.
Access stays in place until the support team confirms it has been removed.
Still have a question?
Start a chat in the mobile app or web portal. The support team can resend the form or check who currently has access.
