Accepted IDs and common questions about IDs

Your government-issued ID is used to verify your identity and ensure that only you can access your medical records.

Why do I need to upload my ID?

To comply with healthcare regulations and attend your appointments, you must have a valid photo ID on your Circle Medical account. This article covers everything about IDs, including: the types of government-issued IDs that you can upload to your Circle Medical account, what to do if your ID has expired and you have a new one, and temporary IDs.


What types of ID can I use?

Circle Medical accepts most valid, government-issued photo IDs. You can upload all of the following ID types using the mobile app:

  • Driver's license

  • State ID

  • Passport

  • Passport card

  • Permanent Resident card


When should I upload my ID?

Please upload your ID as soon as you create your account. To ensure a smooth check-in, please add your ID at least one hour before your appointment starts.

If you have an upcoming appointment and your ID is not on file, you will be prompted to upload the document when checking in.

Important notes:

  • You must upload your ID yourself using the mobile app. Our Care Team cannot upload official IDs on your behalf for security reasons.

  • You can also upload your ID via the web portal exclusively during appointment check-in.


What do I do if my ID has expired or I need to change my ID?

To update an expired ID or change your ID, your Care Team must first remove the existing ID from your profile. You can only upload a new ID once the old one is deleted by the Care Team.

Steps to change your ID on your Circle Medical account:

  1. Start a chat to request that the Care Team remove your current ID.

  2. Once the Care Team confirms that your previous ID has been removed, upload your new ID via the mobile app.


Important callouts about IDs and appointments

  • A valid government-issued ID is needed to attend your appointment.

  • Temporary IDs must be shared via chat so that the Care Team can upload to your chart.

  • Your provider may request that your new ID be uploaded for your appointment.

  • Your provider may cancel your appointment if you don't have a valid government-issued ID on your Circle Medical account at the time of your appointment.

What if my ID verification fails?

If your ID isn't approved, you will be prompted to retake the photos. For the best results, ensure you are in a well-lit area and that the text is clear.
