Confirm your account and personal details
Before your appointment, make sure your account information is complete and up to date. This includes verifying your ID and adding your insurance details, if you have any. Double check your contact information too, like your address and preferred pharmacy.

What to have handy
Keep these items nearby. Your provider may ask for them during your appointment:
Previous medical records
A list of medications you take, with dosages and instructions
A list of any allergies, including medication or ingredient allergies
Names and contact information for your previous doctors, offices, or clinics
Have these ready ahead of time? Upload them on the Circle Medical mobile app or web portal by navigating to the Health tab.

Pre-appointment questionnaires
Depending on your appointment type, you may have a few short questionnaires to complete. You'll fill these out during check-in. They help your provider prepare the best care plan for you.
