You can upload them directly through the Circle Medical app: go to the Health Records tab, tap “Prior Medical Records,” then select the “Add / Request Record” button.
Once submitted, records are typically processed and added to your chart within 3–5 business days for provider review.

What to Expect at Your First Appointment
During your first visit, your provider will evaluate your symptoms, discuss your concerns and goals, and review your medical records if available. While not required, we recommend bringing the following items to your appointment:
Previous medical records, including your most recent Annual Wellness Exam (must be completed within the last 12 months)
A current list of medications you’re taking
Any known allergies
Contact details for your previous medical providers
Appointment costs will vary based on whether you’re using insurance or paying out of pocket. Coverage eligibility depends on your specific insurance plan, so we recommend contacting your insurer directly before booking to confirm coverage.

*Over the last 12 months, 75% of patients with accepted insurance have paid $40 or less per appointment. Costs are estimates and may change according to your deductible. For a definite price, call your insurance company.

At Circle Medical, a thorough, evidence-based ADHD assessment typically takes 2–3 appointments. During these visits, your provider will work closely with you to review your symptoms, concerns, and detailed health history. Sometimes, symptoms that seem like ADHD may actually have other causes and require different treatment.

Establishing Care with AWEs
To begin ADHD care with Circle Medical, you must have completed an Annual Wellness Exam (preventative or physical exam) within the past 12 months. This is sometimes called an Annual Physical. This exam helps your provider understand your overall health and ensures a safe and accurate diagnosis.
If you have already completed an AWE in the past year:
Share these records by uploading them directly in the Circle Medical app or in our web portal (instructions provided in the “Sending Us Your Medical Records” section below).
As a reminder, the annual wellness, or physical, exam clinical note documentation should include:
Documentation that the visit was for an annual wellness exam or physical exam, dated within the past 12 months.
A full clinical note from the visit that includes, but is not limited to:
Vital signs
Any cancer screenings or preventative care
Physical exam findings
Medical history
ICD-10 diagnosis codes related to the care provided.
If you haven’t completed an AWE in the past year:
You can easily schedule one with your Circle Medical provider, either in person or virtually.
Helpful Tip
Even if you have received an ADHD diagnosis in the past, an Annual Wellness Exam is still required to establish care on our platform.

Transferring ADHD Care to Circle Medical
If you've already been diagnosed with ADHD and are on a treatment plan, we’ll aim to continue your care with as little disruption as possible. Here’s what you should know:
If you were diagnosed previously through a virtual or telemedicine evaluation, a re-evaluation with a Circle Medical provider is required.
All patients must complete our ADHD intake appointment, where your provider will review:
Your state’s Prescription Drug Monitoring Program (PDMP) report
Your previous ADHD records to confirm previous diagnosis and treatment plan
Your full medical history and ADHD treatment plan
Documentation of your Annual Wellness Exam (valid only if completed within the last 12 months)

To help streamline your intake process, you can submit your medical records before your first appointment using one of the following methods:
Upload records or request records from another facility via:

Our web portal
How to Share or Upload Records in the Mobile App:
Open the Circle Medical app.
Tap the “Health Records” tab on the screen located along the bottom navigation bar.
Select the “Prior Medical Records” option.
Tap the blue “Add / Request Record” button.
Choose “Upload” OR “Request from Prior Facility,” depending on your needs.
How to Upload Records through our Web Portal:
Navigate to the following page on your browser: https://my.circlemedical.com/records.
Click on the “Health” tab towards the top of the screen.

If you're having trouble submitting records through the app, you can:
Message the Patient Care team through the app, or

After Your First ADHD Appointment
After Your First ADHD Appointment
After your ADHD evaluation is complete, which typically occurs over 2-3 appointments, your provider will discuss the next steps with you. These steps may include:
Treatment options, such as medication or therapy,
Please note that monthly follow-ups are required for any ADHD Controlled Substances prescribed at Circle Medical.
Additional testing (e.g., lab work or referrals), or
Care coordination with specialists or psychiatrists, if needed.
Our goal is to provide care that is personalized, safe, and appropriate for your needs. We are a primary care practice. Depending on your health needs and your provider’s clinical judgement, in some cases, we may recommend seeing a specialist for more focused or frequent care.
