Details on our approaches to supporting patients in our primary care setting.

Updated over 5 months ago

We care deeply for you. You, your care, and your safety matter.
If you are in a mental health crisis, or are experiencing suicidal thoughts, get help right away via the Suicide Prevention Lifeline (800-273-8255) or by texting HOME to the Crisis Text Line at 741741. These services are free and confidential.

At Circle Medical, our providers are board-certified in primary care and are not always equipped to support patients with certain mental health histories. In order to support our patients and ensure your safety, we may recommend establishing care with a local mental health specialist. Patients who are ineligible for specific mental healthcare through Circle Medical can continue to see our providers for primary care services.

Current Medications & Treatments

Patients are generally ineligible for mental healthcare services (including treatment for ADHD, anxiety, and/or depression) at Circle Medical if they are actively engaged in the following treatments (or have been in the last three months):

  • Chronic Daily Opioids: Patients taking opioids every day as prescribed by a primary care provider are not eligible. Circle Medical does not prescribe narcotics or opioid medication. We may accept patients on a stable chronic pain plan managed by a specialist for over a year. Additionally, patients on a stable buprenorphine treatment for over a year are eligible for mental health services. Patients should be prepared to discuss these medications with their Circle Medical provider.

  • Chronic Daily Benzodiazepines: Examples include alprazolam, lorazepam, or clonazepam. Only occasional, as-needed treatment is permitted.

  • Chronic Nightly Sedatives/Hypnotics: Examples include zolpidem or dayvigo. Only occasional, as-needed treatment is permitted.

  • Methadone Treatment Program: To support patient safety and clinical care coordination, patients currently engaged in a methadone treatment program are not eligible for behavioral healthcare.

  • Kratom: Patients currently using Kratom are not eligible.

  • Stimulants Higher than FDA-Approved Doses: Patients with an existing ADHD diagnosis and a treatment plan that requires stimulants higher than FDA-approved dosages are not eligible for mental healthcare through our practice.

To ensure our patients' safety and adherence to our clinical policies, Circle Medical providers will perform a review your state's Prescription Drug Monitoring Program (PDMP) to review any controlled substance prescriptions within the past year

Current & Prior Psychiatric History

Our mission in fostering accessible care includes a focus on ensuring patients are well-supported and have care that meets their specific needs. Our providers may find that patients with the following medical histories may be better suited for focused behavioral healthcare outside of a primary care setting:

  • Multiple Psychiatric Medications: Patients who have trialed multiple psychiatric medications without symptom relief may be advised to consult a psychiatrist outside of Circle Medical.

  • Specific Psychiatric Diagnoses: Patients who have been diagnosed with bipolar disorder, schizophrenia, mania, paranoia, or C-PTSD will need to establish care with a psychiatrist outside of Circle Medical to meet the needs of these conditions.

  • History of Substance Use Disorder or Alcohol Use Disorder: Patients with a history of substance use disorder, including but not limited to addiction or misuse of alcohol, prescription drugs, or illicit drugs, must have been abstinent for at least 12 months in order to be considered for mental health treatment at Circle Medical. Your Circle Medical provider may order a urine drug screen before starting treatment or may recommend that you seek care with a psychiatrist.

  • Suicidal Ideation or Thoughts of Self-Harm: Patients with active suicidal thoughts will be referred to their local emergency room for care.
