Establishing Care
At Circle Medical, we believe healthcare works best when it’s personal, continuous, and informed. That’s why we ask patients to establish care with us, so we can provide safe, effective, and tailored support for your unique health needs.
What Does It Mean to “Establish Care”?
Establishing care allows your provider to understand your full health history and develop a more complete picture of your current health. This is often required before prescribing certain medications or starting treatment plans.
You can establish care with Circle Medical in one of two ways:
Complete an Annual Wellness Exam (AWE) with Circle Medical
You can schedule a virtual or in-person AWE with us. Look for appointment types labeled “Video Wellness” or “Annual Wellness.”
Your exam must have taken place within the past 12 months to be considered valid.
OR
Transfer Records from an Outside Primary Care Provider (PCP)
If you've already had an annual physical in the last year, you can send us your records from your PCP.
These records must clearly show that a wellness exam or physical was completed.
Why Do We Require You to Establish Care?
Wellness exams are a cornerstone of preventative care and provide a foundation for safe treatment. They give us a reliable snapshot of your health, including:
Lab results (e.g., cholesterol, iron, and vitamin D levels)
Vitals (e.g., blood pressure, heart rate)
Medication list and any chronic conditions
Medical, social, and family history
Health screenings and preventative care
How We Collaborate With Your Other Providers
At Circle Medical, we don’t work in a silo, we collaborate with your broader care team. Here's how:
Coordinated Care
If we start or adjust a treatment (like changing medications), with your consent, we can notify your PCP so everyone stays informed.
Shared Records
When needed, with your consent, we securely share medical records with your other providers to ensure consistency and avoid risks.

Important Note on Insurance
Many health plans cover AWEs. As the name suggests, most plans only cover one annual exam per year. However, it's crucial to understand that discussions on non-preventative issues during the exam may incur additional costs, depending on your insurance plan.
For a better understanding of your plan's eligibility criteria, speak to your insurance provider directly.
