Make the Most of Your First Appointment

Confirm details and gather essential records for your initial visit.

Thank you for booking an appointment with us! We're excited to partner with you on your care journey. Here are some tips to make the most out of your first visit.

Confirm your personal information and account details

Ensure all of your information is completed and up to date in the app. These steps include verifying your ID and uploading your health insurance details, if you have any. Double-check that your contact information and preferences, such as your address and preferred pharmacy, are correct.

What to have handy

It's a good idea to have quick access to the following items, as your provider might ask for them:

  • Previous medical records, including the full notes from your most recent Annual Wellness Exam (AWE), also known as an annual physical.

  • A complete list of any medications you take, along with instructions and dosages.

  • A list of any allergies you have, including medications or ingredients.

  • The names and contact information of your previous doctor(s) and their offices or clinics.

If you already have these items available and would like to share them in advance of your appointment, you can upload them directly through the Circle Medical mobile app or the web portal (Health Records tab > "Prior Medical Records" option > "Add / Request Record" blue button).

Pre-visit questionnaires

Depending on the type of appointment you booked, there may be questionnaires to fill out. These short questionnaires can be completed during the appointment check-in step and help your provider prepare the best care plan for you.
