Circle Medical may be able to bill your appointments through your HMO plan, but there are a few things you’ll need to do first.
Once you complete these steps to get your approved HMO referral documentation, the Care Team can verify the referral information with your insurance, and help book your appointment:
Find your assigned Primary Care Provider (“PCP”)
Ask your PCP to request a referral to Circle Medical through your HMO plan
Wait for your HMO plan to approve the referral. Most patients receive their approved referral document within 1–2 weeks.
Get your approved referral document from your insurance plan, and confirm that it includes:
Your referral number
Effective start and end dates
PCP information
Total approved visits
Share your referral document with Circle Medical on the mobile app or web portal
Start a new chat, select “HMO Referral,” and follow the prompts to share your referral document.
