Why do I need to upload my ID?

Your government-issued ID is used to verify your identity and ensure that only you can access your medical records. To comply with healthcare regulations and attend your appointments, you must have a valid photo ID on your account.


What types of ID can I use?

Circle Medical accepts most valid, government-issued photo IDs, including:

  • Driver License

  • State ID

  • Passport

  • Passport Card

  • Permanent Resident Card


When should I upload my ID?

Please upload your ID as soon as you create your account. To ensure a smooth check-in, please add your ID at least one hour before your appointment starts. If you have an upcoming appointment and your ID is not on file, you will be prompted to upload the document when checking in.

Important note: You must upload your ID yourself using the mobile app. Our Care Team cannot upload official IDs on your behalf for security reasons. You can also upload your ID via the web portal exclusively when checking in for your appointment.


What do I do if my ID is expired?

If your ID has expired, please start a chat to request that our Care Team remove the your expired ID from your account. Once it is deleted, you can upload your valid replacement.

A valid ID is needed to attend your appointment. Temporary IDs must be shared via chat, so that a Patient Care Advocate can upload it to your chart. Your provider may request for your new ID to be uploaded for your appointment.


What if my ID verification fails?

If your ID isn't approved, you will be prompted to retake the photos. For the best results, ensure you are in a well-lit area and the text is clear.
