How to request your medical records

Learn how to get your medical records sent to you via your Care Team.

Whether you’d like to receive your latest appointment note, sleep study results, or all of your Circle Medical health records, your Care Team will help you in this process. This article guides you through the steps to request your medical records.

Requesting your medical records on the mobile app or web portal

  1. Log in to your Circle Medical profile.

  2. Start a chat with your Care Team.

  3. Select ‘Medical Records’ and follow the prompts.

  4. Let the Patient Care Advocate (PCA) know the specific records you’d like to request.

  5. Verify your identity with the PCA in the chat.

  6. The PCA will then send you a secure link to your medical records.

  7. Select the link and download your medical records as a PDF document.

Important note: For your security, the link to your records expires in seven days. If you choose to receive your records by email, the document is password protected. You will find specific instructions on how to access your records within that email.

Pending appointment notes

Your appointment notes are typically finalized by your provider within 24 hours of your visit. Once finalized, these records are accessible to you on your profile. If your appointment notes aren’t showing after 24 hours, please start a chat with your Care Team so they can look into the delay for you.
