Preparing for an ADHD Intake Appointment

What to expect during an initial appointment exploring potential ADHD symptoms.

We're here to support you in your physical and behavioral health journey, and we're glad you chose to partner with us for your care. While no preparation is required for an initial appointment, we want to help you get the most out of your first visit with us.

Timeframe for Evaluation

At Circle Medical, it takes an average of 2-3 appointments to complete a thorough, evidence-based ADHD assessment. During these visits, your Circle Medical provider will partner with you to review any symptoms or concerns you have as well as your detailed health history. Sometimes, symptoms that may initially appear to be related to ADHD have other underlying cause(s) and may be better treated differently.

Establishing Care and Annual Exams

In order to establish care at Circle Medical, it is required to have an annual physical (annual preventative visit or annual wellness exam) completed within the last 12 months. This is to ensure we have your complete medical history.

If you have not had an annual preventative visit in the last 12 months, you can easily schedule one with your Circle Medical provider. These exams may be performed in-person at one of our clinics or virtually.

If you have already seen another doctor or clinician for an annual preventative visit during the last year, you may send your medical records to [email protected] prior to your first appointment. If you don't have them, you may request these records directly from the provider or clinic that performed the exam. The exam will need to be verified with your provider upon receipt of medical records to ensure it meets certain criteria; the exam should include your blood pressure, pulse, height, weight, and documentation of a heart and lung exam.

Treatment Options

Your Circle Medical provider will review potential treatment options with you following your evaluation appointments. Treatments, diagnoses, and options depend on the results of your evaluation and your medical history. For select patients, additional testing or evaluation may be necessary. This may include lab work, referral or care coordination with outside specialists, or a recommendation to seek care with a psychiatrist.

Your first intake appointment will help your provider determine if you are a good candidate for mental health care at Circle Medical. While our goal is to help every patient in our primary care setting, it may be suggested that you begin care with a specialist or a provider that can see you in a more frequent or accessible setting. The safety and well-being of our patients will always be our top priority.

What to Bring to the Visit

While not required, the following items are helpful to have ready for your appointment:

  • Any relevant medical records, including any prior mental health visits, your last annual wellness exam (also known as an annual physical or preventative visit), and your vaccine records.

  • A list of your current medications, including instructions and dosages.

  • A list of any allergies you have.

  • The name and contact information of your previous health clinics.

  • A recent reading (within the last year) of your blood pressure and heart rate.

  • Your current height and weight.

Transferring ADHD Care to Circle Medical

If you have been previously diagnosed with ADHD and are on an established treatment plan, our goal is to limit the disruptions in your ongoing treatment as much as possible. Please be aware of the following Circle Medical policies:

  • If a telemedicine platform performed your initial ADHD diagnosis, you will be required to undergo a reevaluation by a Circle Medical provider.

  • Regardless of prior diagnosis, every patients must undergo an initial intake appointment, during which your Circle Medical provider will review:

    • State-specific Prescription Drug Monitoring Program(s) to review any controlled substance prescription history.

    • Your complete medical history and documented symptoms.

    • Your previous ADHD journey (including diagnosis, treatment, and other notes).

  • Annual preventative visit (annual physical or annual wellness) records are required to begin care for ADHD. These records provide you and your provider a more comprehensive overview of your medical history, symptoms, and other conditions to aid in clinical decision-making and goal planning. Annual visits must be completed within the last 12 months.

    • If you have not had an annual preventative visit in the last 12 months, you can schedule one as an additional visit with your Circle Medical provider.

    • If you have had an annual preventative visit within the last 12 months, send these records to [email protected] before your appointment. If you don't have them, you may request these records from the clinic that performed the exam.

    • The annual preventative exam will need to be verified with your provider upon receipt of medical records. The exam should include blood pressure, pulse, height, weight, and documentation of a heart and lung exam.
