An authorized user is someone who you, as a patient, choose to grant access to your sensitive health information. This could be a family member, caregiver, or a close friend whom you trust. As an authorized user, this person will have the ability to view and manage aspects of your health information, ensuring that they can assist you in your healthcare journey.

How to Add an Authorized User

  1. Contact the Care Team: To begin the process of adding an authorized user, please get in touch with our Care Team via chat. Our team is available to guide you through this process and answer any questions you might have.

  2. Complete the Authorization Form: You will be required to fill out a form to officially authorize the individual of your choice. This form ensures that your health information is shared securely and in accordance with your wishes.

  3. Provide Information about Your Chosen User: During this process, you will need to provide specific information about the person you are authorizing. This includes their full name, relationship to you, and any other relevant details necessary for this authorization.

Revoking Authorization

You have complete control over your health information. You can revoke the authorization granted to an authorized user at any time. To do this, you can:

  • Inform the Care Team via Chat: Reach out to our Care Team through the chat function to let them know you wish to revoke an authorization.

  • Inform Staff or Your Provider During an Appointment: You can also inform your healthcare provider during a regular appointment about your decision to revoke the authorization.
