Your privacy and security are extremely important to us—that’s why we ask all patients to verify their ID. This helps us make sure it’s really you attending the visit and protects against fraud. It also helps us follow healthcare regulations and standards to keep your information safe.

  • State-issued driver's license

  • State-issued non-driver ID card

  • Permanent resident card

👍 Helpful Tips

  • Take photos in a well-lit area.

  • Avoid glaring and the use of flash.

  • Make sure your ID is valid and up to date.

  • Place your ID flat on a dark, solid surface.

  • Hold your device steady while taking the photo.

Uploading your ID

On the Mobile App

During the appointment check-in step, you will be prompted to verify your ID. Follow the screen prompts by taking pictures of the front and back of your ID before submitting.

If your ID isn’t verified instantly, you will be able to check on the status of the verification in the "Account" page. This page can be accessed by tapping the "Settings" icon in the upper right corner of any page.

If you happen to attend your appointment before this is resolved, your provider may ask you a few questions if the results of the ID verification were inconclusive.

On the Website

When you have an appointment booked in the future, the link to verify your ID in the app will appear below the upcoming appointment.

Re-Verifying your ID

Sometimes, you may need to re-verify your identity if your ID is expired, if there's a problem with your original form of identity, or for another reason.

You can manually verify your ID by tapping the "Settings" icon in the upper right corner of any page in the app. Scroll to the "ID Verification" field, under the "Account" section and tap to resolve the conflict or upload your ID.
