What information is needed for a prior authorization?

To submit a prior authorization (PA) request, your provider needs the following information from your insurance card:

  • Member ID

  • RxBIN

  • PCN

  • RxGroup

How do I provide this information if I have it?

If you need a prior authorization and have the necessary information, follow these steps to send it to your Care Team:

  1. Find your prescription or pharmacy benefits card.

  2. Take clear photos of the front and back of the card.

  3. Start a chat on the mobile app or web portal, and send the images there.

What do I do if I don’t have my insurance card, or if some information is missing?

  1. Call your insurance and ask for your Member ID, RxBIN, PCN, and RxGroup.

  2. Start a chat via mobile app or web portal, and send the information there.

Tip: If a Patient Care Advocate is assisting you with a prior authorization, be on the lookout for updates in your chat!

Is it possible to have a prior authorization expedited?

PAs are usually only expedited if the timeline for a standard prior authorization could put your life at risk.

In some cases, if you need your medication sooner, you may pay out of pocket and request reimbursement if the PA is later approved.

Tip: If you’ve self-paid for your medication, keep your receipt from the pharmacy in case your insurance requests it.
