Uploading Your Insurance Card

To help us provide you with seamless care and avoid billing issues, we need a copy of your insurance card to verify your coverage.

We're committed to making your care prompt and hassle-free. Providing a copy of your insurance card helps us confirm your coverage and benefits ahead of your visit. This allows us to focus on your care—not paperwork—and helps you avoid unexpected costs or delays in referrals.

Uploading Your Insurance Card Images

You can upload images directly through the mobile apps or web portal, either through the "Billing" area, or at appointment check-in.

To upload your cards outside of check-in, visit the "Billing" tab located in the primary navigation. From there, select your primary insurance, and follow the prompts to add your insurance card images from the Insurance Details page.

If you have not already uploaded your images and have an upcoming appointment, you will be prompted to upload a copy when checking in for your appointment.

If the upload is unsuccessful or you're encountering technical details, send pictures of your card to us via the in-app chat, so we can upload them to your account.

Some Important Notes on Insurance

Verify your network coverage, medical benefits, and deductible with your insurance provider before booking an appointment to eliminate any misunderstandings or unexpected charges.

All providers at Circle Medical are unable to provide care to Medicaid or Medi-Cal patients, regardless of how you choose to pay for your appointments. If you use these services, it is important to see the provider assigned by your program to avoid losing coverage.
