Scanning Insurance Cards

Scanning Insurance Cards

Automatically add your insurance information to your chart.

Updated this week

We're committed to making your care prompt and hassle-free. To help us do that, we may need images of your insurance card for certain steps, such as referring you to a specialist.

We accept most PPO plans from Aetna, Anthem Blue Cross, Blue Shield of California, Cigna, and United Healthcare. We also accept HMO plans from the San Francisco Bay Area where the medical home is Brown and Toland or Hill Physicians.

Mobile App Check-In

During the appointment check-in step, you will be asked to scan your insurance card. Follow the prompt on the phone to center your card in the frame, and then confirm the details on the screen that follows. If the scan is unsuccessful, we may ask you at a later date via chat to send pictures of your card.

Uploading Manually

You can also upload images directly through the Account page, which can be accessed at any time by pressing the account icon in the top right corner of the screen. Scroll to the Insurance section and tap on "Change" to update your insurance details.

Some Important Notes on Insurance

Verify your network coverage, medical benefits, and deductible with your insurance provider before booking an appointment to eliminate any misunderstandings or unexpected charges.

All providers at Circle Medical are unable to provide care to Medicaid or Medi-Cal patients, regardless of how you choose to pay for your appointments. If you use these services, it is important to see the provider assigned by your program to avoid losing coverage.

At our practice, we're committed to providing quality care to those who need it. Please provide accurate personal information. The use of false information will result in immediate termination from our platform.
