Your privacy is our top priority. Our goal is to ensure your personal health information is secure, accessible, and manageable. We understand that you may need a trusted person to help make medical decisions. We have a secure process to add an authorized user.


What is an authorized user?

An authorized user is someone who you choose to grant access to your sensitive health information. This could be a family member, caregiver, or a friend that you trust.


What can authorized users see and do?

An authorized user can have the ability to view and manage specific aspects of your health information, ensuring that they can assist you in your healthcare journey.


How to add an authorized user

  1. Contact the care team: To add an authorized user, reach out to our Care Team via chat. They will guide you through this process and answer any questions you might have.

  2. Complete the authorization form: Our team will send you a Patient Information Access Authorization Form to your Circle Medical registered email for you to complete.


Why do I need to complete an authorization form?

An authorization form clearly identifies the authorized user and lets you decide the extent of access they have. This can fall anywhere from appointment scheduling/reminders, billing records, full medical records access and anything else you wish to specify.


Revoking authorization

Remember, you have complete control over your health information. You can revoke the permissions granted to an authorized user at any time. To revoke or restrict access, please chat with our Care Team via the mobile app or web portal.
