✅ What insurances do you accept?

We accept most major insurances, including most commercial plans from:

  • Aetna

  • Anthem Blue Cross

  • Blue Cross Blue Shield

  • Blue Shield of California

  • Cigna

  • UnitedHealthcare

  • Others

Please note: While we may accept plans within your insurance company, coverage depends on your specific plan and benefits. Even if we’re in-network with some plans within an insurance, other plans may be out-of-network.

Insurance acceptance can also change over time due to updates in contracts between healthcare providers and insurance companies. These changes can affect which plans are considered in-network. We always recommend checking with your insurance provider before your visit to confirm your coverage.


🚫 What insurances are you unable to accept?

We are unable to provide care to Medicaid patients, even if they opt to self-pay. This is because each state’s Medicaid program has its own rules and network, and providers have to go through a separate process to participate.

Please note, some insurances, like UnitedHealthcare, may administer Medicaid plans through contracts with state Medicaid agencies. This may be called a Medicaid Advantage, or Medicaid Managed Care, arrangement. Even though a commercial insurance administers the plan, it is still Medicaid coverage, and the insurance must follow Medicaid rules and benefits.


🕵️ How do I check if my insurance will cover my Circle Medical appointment?

It's important to have a clear understanding of your coverage to avoid any surprise costs. Here are some steps you can take to ensure that you know all the necessary information prior to making an appointment:

  1. Contact your insurance. To confirm your plan's coverage, we recommend reaching out to your insurance provider. The phone number for your insurance is often on the back of your insurance card, listed as the Member Services number. Your insurance can provide you with detailed information about your coverage, including any co-payments or out-of-pocket costs that you may be responsible for. Your insurance plan documents can also provide information about coverage.

  2. Provide your provider's NPI. If asked, reference the NPI number of your provider for an eligibility check. This will help your insurance provider confirm that we are in-network and covered under your plan.

    1. To find your provider's NPI, search the NPI Registry:

      1. Go to the NPPES website.

      2. Use the public search tool, entering your provider's name and potentially their city and state.

  3. Provide the relevant Circle Medical Tax ID. Since we operate in multiple states and have different Tax IDs, your insurance may use this information to locate our practice in their system. Using the right one helps avoid incorrect information about your benefits or coverage.

  4. Consider making one of our providers your PCP. Some insurance providers may require you to select one of our providers as your primary care provider (PCP) to ensure that your charges are in-network.
