We accept most PPO plans from Aetna, Anthem Blue Cross, Blue Shield of California, Cigna, Health Net, and UnitedHealthcare.
We also accept select HMO plans from the San Francisco Bay Area where the medical home is Brown and Toland or Hill Physicians.

It's important to have a clear understanding of your coverage to avoid any surprise costs. Here's some steps you can take to ensure that you know all the necessary information prior to making an appointment:
Contact your insurance provider. To confirm your plan's coverage, we recommend reaching out to your insurance provider. They can provide you with detailed information about your coverage, including any co-payments or out-of-pocket costs that you may be responsible for.
Provide our NPI and Tax ID. If asked, reference our NPI number, which is 1386855120, and our Tax ID, which is 474883537, for an eligibility check. This will help your insurance provider confirm that we are in-network and covered under your plan.
Consider making one of our providers your PCP. Some insurance providers may require you to select one of our providers as your primary care provider (PCP) to ensure that your charges are in-network. If this is the case, you can provide our chief medical officer, Dr. Nicole Tsang, DO, as your PCP.

You may see claims under Dr. Nicole Tsang, our chief medical officer, even if you saw a different provider. Don't worry - you can still continue seeing your existing Circle Medical provider.
We are unable to provide care to Medicaid or Medi-Cal patients, regardless of how you choose to pay for your appointments. If you use these services, it is important to see the provider assigned by your program to avoid losing coverage.
