We accept most PPO plans from Aetna, Anthem Blue Cross, Blue Shield of California, Cigna, Health Net, and UnitedHealthcare.

We also accept select HMO plans in the San Francisco Bay Area.

  1. Contact your insurance provider. To confirm your plan's coverage, we recommend reaching out to your insurance provider. They can provide you with detailed information about your coverage, including any co-payments or out-of-pocket costs that you may be responsible for.

  2. Provide our NPI and Tax ID. If asked, reference Circle Medical's NPI number, which is 1134596091, and our Tax ID, which is 474883537, for an eligibility check. This will help your insurance provider confirm that we are in-network and covered under your plan.

  3. Consider making one of our providers your PCP. Some insurance providers may require you to select one of our providers as your primary care provider (PCP) to ensure that your charges are in-network.
